Volunteer Awards

Guidelines

The Governor's Volunteer Service Award Nomination Form consists of three sections:

  1. The Nomination Form
  2. The Nomination Statement
  3. The Nomination Statement Adendum

All sections of the form must be completed. Incomplete forms will not be accepted by the NC Commission on Volunteerism and Community Service.

Nominations are selected based on merit - without regard to race, religion, gender, national origin or physical/mental disability.

Additional Guidelines

  • All nominations must be submitted on the official 2009 Governor's Volunteer Service Award nomination form. Photocopies of the form are acceptable.
  • If the nominee is a group or business, the name of one contact person must be provided. That individual will be the person who will attend the ceremony and accept the award on behalf of the group or business.
  • Each county may submit a maximum of five (5) nominees. A husband/wife, family, group or team who volunteer as a unit may be considered as one nominee.
  • One of the nominees may be identified (by the county) for consideration for the North Carolina Medallion Award.
  • Evaluation within the NC Commission on Volunteerism and Community Service is based ONLY on the contents of the nomination form, nomination statement and nomination statement addendum.
  • Videotapes, cassettes, display materials, scrapbooks, books, photographs, etc. should not be submitted to your local County Coordinator.
